The primary function of the medical Scribe is partnering with the Physician/Provider to create and maintain each patient’s medical chart. The Scribe adds electronic health data into the record in real time as patients interact with the Physician/Provider. While the Scribe listens to the interaction from a remote on-site or off-site location, the data he/she enters appears on a screen viewed by the Physician/Provider.
The Scribe and Physician/Provider utilize two-way communication to ensure understanding and accuracy of essential information. The Scribe records symptoms, diagnoses and medical codes to ensure proper billing. He/she acts as the Physician/Provider’s liaison with laboratories and staff, ordering tests, diagnostics, or shots. He/she prints out the patient educational material which becomes a part of the patient’s permanent medical file. Following each patient encounter, the Physician/Provider reviews and authenticates the chart.
