Community Health Specialist Position Summary
The Community Health Specialist for the Mobile Mammogram Van is responsible for providing culturally appropriate health education and information, community health outreach, helping people get care, giving informal advice about health behaviors such as chronic-disease self-management, advocating for individuals and community health needs, and providing some direct services such as blood pressure screenings. The Specialists assists with outreach to schedule patient appointments and follow-up related to the Mammogram Van and Wellness Program.
This role coordinates the communications, registering and greeting patients, and maintaining financial information.. Following assessments, the Community Health Specialist works with the patient to prioritize needs and address identified needs such as transportation, utility assistance, housing, financial assistance, health insurance coverage, and links to a primary care provider. The Specialist is a key part of a multidisciplinary healthcare team that includes coordinators, nurses, social workers, community health workers, and nurse practitioners.
The Specialist serves as a liaison with community agencies and referral services, interfaces with external agencies, prepares and maintains timely and accurate documentation of all activities in accordance with established standards and procedures, as well as enter data in platforms.

Position Requirements
Education
* High School Diploma or equivalent is required.
* Bachelor's degree in relevant field is preferred.
License and/or certification
* Certified as a Nursing Assistant (CNA) is preferred
Experience
* Three (3) years clerical/administrative experience is preferred.
* Experience in a healthcare setting and medical terminology preferred.
* Experience working with Microsoft Office computer programs, practice management systems and spreadsheets preferred.
Community Health Specialist Schedule
* Varies schedule, primarily day shift.
* Evenings and weekends may be required as needed.
Community Health Specialist Benefits
* At Tidal Health, team members working at least 36 hours per pay period based on 12-hour shift schedules or at least 37.5 hours for non 12-hour shift schedules and part-time team members working at least 30 hours or more on weekends only are eligible for benefits.
