Position SummaryThis nurse will assess, plan, implement, evaluate, and document nursing care of patients in accordance with policy and professional standards. The role is accountable for the quality of nursing services delivered by self or others who are under their direction.
Core Functions- Assess patient physical, psychological, social, spiritual, educational, developmental, and cultural needs; review history, diagnostics, and lab data; report abnormalities to physicians.
- Formulate and update individualized plans of care and discharge plans, considering all patient needs.
- Implement plans through direct care, coordination, delegation, and supervision of the care team; ensure compliance with policies and regulatory requirements.
- Evaluate patient and family responses to care and revise plans as necessary; evaluate progress toward goals.
- Document assessment, planning, implementation, and evaluation in the patient record; maintain legible, timely documentation.
- Provide evidence‑based care; participate in research, unit leadership, education, orientation, mentoring, and quality improvement initiatives.
- Collaborate with all levels of staff, external providers, and community agencies to support patient and family care.
- Supervise staff and workflow in certain roles; support other health care providers and offer spiritual support when needed.
- Current, valid RN license in the state of practice (or temporary/interim/compact license).
- BLS certification required; additional certifications may be required.
- Experience in the clinical area of application is desired; one year of acute care experience in the past 12 months required for Registry & Travel positions.
- Home Health nurses must possess a valid driver’s license, active auto insurance, and personal vehicle.
- Behavioral Health roles require appropriate Arizona or Colorado fingerprint clearance cards;
Criminal History Affidavit required.
- Bachelor’s degree preferred.
- Professional certification preferred.
