Licensed Practical Nurse - LPN

Job Description Responsibilities

The LPN contributes to the assessment, care plan, implementation of nursing care, and evaluation of nursing care including CIWA/COWS etc. assessments for assigned patients within the LPN scope of practice through data collection, monitoring, observing, recording and communicating patient responses to the supervising Registered Nurse. The LPN performs nursing care consistent with the LPN scope of practice including administering treatments, medications, and procedures under the delegation and supervision of an RN.

Minimum Education: Graduate of an accredited practical or vocational nursing program

Preferred Experience: 1 year LPN experience with at least 6 months in a behavioral health setting

Required Certification/ License: Valid LPN License in the state of Arizona or LPN/LVN License issued in a Nursing Licensure Compact state with multi-state privileges
