• Coordinating delivery of services to guests in collaboration with the facility’s team members.
• Monitoring and documenting the effectiveness of treatment provided, coordinating the admission process and determining appropriate post‑discharge services with the interdisciplinary team (IDT).
• Coordinating and updating care plans; attending meetings when requested by guests or deemed appropriate by the IDT.
• Participating in daily IDT meetings and providing clinical review as necessary.
• Completing assigned sections of discharge plan instructions and reviewing clinical post‑discharge instructions with guests upon discharge.

• Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Must possess a current, unencumbered, active license to practice as an RN or LPN/LVN through the State Board of Nursing.
• Must have a minimum of two (2) years’ experience in a hospital, nursing care facility, or other related health‑care facility.
