Physical Therapist – Home HealthAs the Physical Therapist in Home Health, you will complete assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain and other medical conditions. Based on this assessment, you will work to determine a treatment plan, perform interventions aimed at improving and enhancing the patient’s well‑being, and evaluate progress.
Primary Responsibilities- Evaluate the patient’s functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
- Provide services within the scope of practice as defined by state laws, in accordance with the plan of care and in coordination with other health‑care team members.
- Observe and report the patient’s response to treatment and any changes in condition to the supervising nurse and/or the physician.
- Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
- Instruct and inform team members, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Visit patients according to the Plan of Treatment, complete a progress note for each visit, and submit progress notes per LHC policy.
- Complete all patient evaluations and develop the PT plan of care within state‑specific guidelines.
- Report outcomes of evaluation, goals, and anticipated projected frequency of care.
- Current Physical Therapy licensure in state of practice.
- Current CPR certification.
- Current driver’s license and vehicle insurance, or access to a dependable vehicle or public transportation.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
Pay is based on factors including local labor markets, education, work experience, certifications, etc. In addition to salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution. The hourly pay for this role ranges from $34.23 to $61.15 per hour based on full‑time employment. We comply with all minimum wage laws as applicable.
