Admission​/Discharge Registered Nurse

Are you passionate about patient care and thrive in a fast-paced environment? Regional West is seeking a Med-Surg Admit and Discharge Nurse to join our dedicated team and ensure smooth transitions for patients entering and leaving our medical-surgical unit.

You’ll play a critical part in the patient experience by coordinating admissions and discharges, ensuring accuracy, and providing compassionate care during these important transitions. Plus, you’ll support your fellow nurses by assisting with patient care tasks when needed, fostering teamwork and collaboration.

• Complete admission assessments and initiate care plans
• Educate patients and families on treatment plans and discharge instructions
• Coordinate with physicians, case managers, and interdisciplinary teams
• Ensure accurate documentation and compliance with hospital policies
• Monitor patient status during the admission and discharge processes
• Assist floor nurses with patient care tasks as needed to maintain workflow and quality care
• Support continuity of care and patient safety

• Current RN license
• Strong communication and organizational skills
• Ability to manage multiple priorities in a dynamic environment
• Previous Med-Surg experience preferred
