$2,000 total: $1,000 after 30 days, $1,000 after 6 months of employment.

• Processes patients from assessment to discharge in both medication‑assisted treatment and drug‑free treatment programming.
• Dispenses all medication under procedures set forth by Family Guidance Center, Substance Use Prevention and Recovery, Clinical Sustainability Assessment Tool and the Drug Enforcement Administration.
• Monitors the appropriateness of methadone and medication dosages.
• Monitors the physical & mental health well‑being of patients and documents findings.
• Must be available to travel if needed.

Pay Rate: $30.00

Education and/or

Experience:

At least two years in hospital or agency setting.

Certificates, Licenses, Registrations: L.P.N. registered in the State of Illinois and certified in CPR.
