Care Manager / Healthcare Advocate

Care Navigators, Inc. and Care Guardians NFP, Inc. are seeking qualified part‑time care managers to join our team. The role is based in Chicago, IL and requires the candidate to live or be willing to travel within the city.

The Care Manager provides concierge, hands‑on service to navigate families and their loved ones through the healthcare system. The Care Manager advocates for clients at home, during doctor visits, in rehab centers and hospitals.

• Care Management
• Perform comprehensive assessments
• Provide care plans with recommendations and resources
• Assist with discharge from rehab/hospital
• Crisis Management
• Healthcare Advocacy
• Medication Management
• Home Care Coordination
• Home Safety evaluations and DME recommendations
• Housing Placement
• Guardianship
• Client visits

• Previous experience in care management, guardianship, or related field
• Strong communication and listening skills (verbal and written)
• Valid driver’s license
• Ability to work autonomously and with a team
• Computer efficacy with Word, Excel and G‑suite
• Recommended but not required:
  License in field of study (RN, SW, or LCSW)
