- Ensure appropriate reimbursement for patient care services.
- Participate in the Patient Assessment Process through completion of MDS and Care Area Assessments (CAAs).
- Assist in development of individualized patient care plans, identify patient needs and projected outcomes per federal and state regulations.
- Contribute to the nursing center interdisciplinary Resident Assessment Instrument (RAI) process, following RAI guidelines for completion, computer encoding and electronic transmission of the MDS.
- Identify changes in patient condition with interdisciplinary team and coordinate scheduling and completion of indicated assessments.
- Assist with Activities of Daily Living training for new hires and ongoing continuing education for existing staff.
- Attend educational programs to maintain knowledge of current RAI/Prospective Payment System regulations.
- Registered Nurse currently licensed by the state and in good standing.
- Previous experience in a hospital, long‑term facility, or other healthcare related facility.
- Prior experience completing MDSs and Care Plans.
- Knowledge of general, rehabilitative and restorative nursing and medical practices and procedures, and laws, regulations and guidelines governing long-term care.
