Overview

Patient Service Area: Effingham, Bryan, and Chatham

Shift: 8-hour shifts

Duration: 13 weeks with possible extension

The Registered Nurse provides skilled nursing care to patients of all ages in their homes, coordinating care with the interdisciplinary team, physicians, and families. Responsibilities include patient assessments, development and management of plans of care, OASIS documentation, medication administration, wound and IV care, pain management, patient and caregiver education, and supervision of home health staff. The RN ensures compliance with regulatory and payer requirements, maintains accurate documentation in Homecare Homebase, and delivers high-quality, patient-centered care while working independently in the field.

• Active state RN license
• 2+ years home health experience
• OASIS and Homecare Homebase experience required
• CPR certification and valid driver’s license
• Reliable transportation and ability to travel locally
