We are seeking a Registered Nurse to join our team. In this role, you will primarily be completing new patient assessments as needed and creating a plan of care for individual patients for non-skilled services. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient s best interest. To succeed, you should have demonstrated knowledge and skill in providing the best recommendations for patient s ADL CNA S task list.
Responsibilities- Coordinate with personal supportive programs to assess, plan, implement, or evaluate patient care plans.
- Document vital signs, potential fall risks, skin integrity notes
- Maintain accurate records at the time of re-assessment periods of patients throughout the year and annual.
- Available on a as needed basis/schedule bases
- Currently licensed Registered Nurse (RN) in state of employment
- Excellent interpersonal and client care skills
