Description Introduction
We have an exciting opportunity for an RN to join Memorial Health Heart & Lung Surgical Care.
This role supports a busy cardiothoracic surgery practice comprised of three CT surgeons and three Advanced Practice Providers. The RN will play a critical role in care coordination, patient education, and clinical triage for heart and lung surgery patients across the continuum of care.

Job Summary and QualificationsFull time (Monday - Friday, days), no nights, no on‑call, no weekends, no holidays.
The Registered Nurse provides ambulatory nursing care focused on care coordination and phone triage for heart and lung surgical patients, including oncology‑related cases. This position supports patients before and after surgery by providing education, follow‑up, and coordination with cardiology and oncology care teams. The role includes a significant administrative and communication component, requiring strong clinical judgment and organizational skills.
What you will do in this role:
Provide phone triage and clinical guidance to heart and lung surgical patients.
Coordinate care and follow‑up for cardiothoracic and oncology patients.
Deliver pre‑discharge education and post‑operative follow‑up calls.
Collaborate with cardiology, oncology, surgeons, and APPs to ensure continuity of care.
Assess patient needs, prioritize concerns, and elevate care appropriately.
Document patient interactions, care plans, and education thoroughly and accurately.
Support patient navigation through complex surgical and oncology care pathways.
Perform other duties as assigned.
What Qualifications you will need:
EDUCATION:
- Graduation from an accredited school of registered nursing is required.
EXPERIENCE:
- Minimum of three years Cardiac RN experience in an acute care or cardiology clinic setting required.
CERTIFICATION/LICENSE:
BLS required upon hire.
ACLS required within 60 days of hire.
Current unrestricted license as a Registered Nurse in accordance with state regulations.
Cardiac Medicine Certification (CMC) through American Association of Critical Care Nursing (AACN) preferred OR
Cardiac Vascular Nursing Certification (CV‑BC) through American Nurses Credentialing Center (ANCC) preferred.
