Full Time Position. Sign‑on bonus: $10,000. Competitive pay range: $32,170.00–$38,926.00.

• $24 to $30 hourly

• Verify plan of care with the RN before each visit.
• Collaborate with physicians and RN to ensure patient needs are met; notify as soon as possible of changes in patient condition.
• Teach patients and caregivers self‑care techniques.
• Check vital signs.
• Review all medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g., Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.

• One year of clinical LPN experience.
• Current LPN license specific to the state(s) assigned.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.
