Position: Oncology Nurse - New Patient Coordinator

Overview

The Oncology Nurse – New Patient Referral & Intake Coordinator plays a critical role in facilitating timely, accurate, and patient centered onboarding of new patients to the ambulatory oncology service line. Strong oncology clinical judgement and prioritization. Familiarity with oncology treatment pathways and staging. Excellent patient-centered communication, interdisciplinary collaboration, attention to detail, follow-through, and organizational abilities. Experience with Meditech.

  • Associates in Nursing - Required
  • Bachelors Degree in Nursing – Preferred
Experience
  • 2 Years Nursing - Required
  • Inpatient & Ambulatory Oncology – Preferred
License & Certification
  • Registered Nurse License with State of Practice – Required
  • OCN - Preferred
  • Basic Life Support (BLS) - Required
Core Job Functions
  • Collaborate with the New Patient Coordination (NPC) team to review and clinically triage new referrals to outpatient oncology from internal and external providers. Partner with the NPC team to ensure timely access to care. Track referral status and follow through until patient is established in care including ensuring lab appointments are scheduled appropriately per provider discretion.
  • Prioritize referrals based on clinical acuity and referring providers referral status as urgent or high-risk cases requiring expedited review or scheduling. Ensure referrals meet system and disease-specific timelines. Escalate complex or time-sensitive cases to providers or leadership as appropriate.
  • Assess referral completeness (diagnosis, pathology, imaging, labs, staging information). Works closely with providers on expectations for new referrals. Communicate with NPCs to obtain missing or clarifying clinical information from external providers.
  • Serve as a point of contact for newly referred cancer patients and families. Provide education on upcoming appointments and the care process. Address patient questions, concerns, and barriers to care through proactive collaboration with the LCRP Supportive Care Services team.
  • Accurately document referral reviews, triage decisions, and patient communications in the EMR.
  • Participate in referral process improvement initiatives. Identify gaps, delays, or safety risks in the referral workflow. Assist with the development and maintenance of referral protocols and triage guidelines.
