Patient Placement Coordinator

• Take calls from physicians or their designees to schedule admissions, obtain demographic information, and communicate with physician offices.
• Notify referring facilities of room assignments to expedite patient transfers.
• Initiate the patient placement process by identifying appropriate beds and coordinating with charge nurses.
• Transfer existing patients to different areas, update patient information in the computer, and communicate bed census and availability to relevant personnel.
• Prioritize room cleaning for environmental control and maintain excellent internal and external customer service.
• Maintain a working knowledge of all hospital and departmental policies, procedures, and professional standards.

• Education: High School Diploma (Preferred)
• Experience: Relevant experience in patient placement or admissions (not specified)
• License & Certification: None Required

• Answer admissions calls promptly and maintain accurate records.
• Input information into the tracking system and print orders to the correct location.
• Follow established policies for patient assignments and coordinate patient movement to manage bed availability.
• Prioritize cleaning needs of patient rooms.
• Perform accurate and timely computer functions for new admissions and transfers, demonstrating knowledge of patient accommodation codes.
• Maintain a log of all calls received and keep the current on‑call list posted daily.
• Contact on‑call physicians in a timely manner with appropriate information.
