The Social Worker uses experience, knowledge, judgment and skills to provide social services to patients and families in both complicated and uncomplicated situations. The Social Worker maintains patient confidentiality and promotes patient rights as supported by agency, state and federal directives. The Social Worker identifies cultural diversity and lifestyle choices and non‑judgmentally accommodates for such differences. Additionally, they provide consultation to other hospice staff and volunteers while consistently demonstrating the Hospice Savannah values and compliance with organization’s service excellence standards.

Graduate of an accredited school of social work with Master’s degree in Social Work required, licensed Social Worker required. Must have a minimum of 2 years social services experience; family work and healthcare experience preferred. Shall adhere to and promote the ethics and values of the Social Work profession using the NASW Code of Ethics as a guide to ethical decision making in social services practice.

Valid driver’s license, automobile insurance and reliable transportation required. Excellent communication skills (written and verbal) and the ability to successfully interact with a diverse patient population, families, physicians, Hospice Savannah staff and the community at-large along with excellent customer service and documentation skills required. Computer proficiency in Microsoft Office, including Word and Outlook, and database knowledge required.

Position requires moderate visual concentration for patient charting/documentation and computer work. This position carries a risk of exposure to infectious diseases and requires skill in standard safety precautions. The Social Worker may be required to assist with ambulating or lifting a patient with or without the assistance of devices in excess of 75 lbs. utilizing proper body mechanics.

A part-time position works a 16‑29 hour work week, rotating days, evenings or weekends, with occasional requests to work additional days/hours/holidays as needed. The Social Worker is required to participate in the rotating on‑call schedule and may be required to provide care in what has been determined by local authorities as a high‑risk crime area and unsafe situations (potential or actual).

This position encounters stressful situations with patients and their families regarding end‑of‑life care. The Social Worker must be able to accurately, timely and concisely document the patient/family encounter using a computer. Although each support services provider may be designated to a specific department/region in the service area, at times they may be required to serve patients outside of that department or region.

This position is required to perform patient visits utilizing their personal vehicle and may require extensive driving.
