Responsibilities- Assist patients in developing problem‑solving skills to respond to challenging experiences in recovery.
- Model effective coping and self‑help strategies and techniques to individuals or groups to overcome personal fears, anxieties, urges, and triggers.
- Assist/support patients in daily decision making, resolution of minor problems, and crisis intervention as necessary.
- Assist patients in identifying personal goals and objectives for recovery and sobriety.
- Assist clinic staff in a collaborative process of identifying programs and environments conducive to recovery.
- Assist patients in creating personal recovery plans.
- Meet with patients to provide advice and support in accordance with treatment plans.
- Assist/coordinate patient support groups on a regular basis.
- Assist patients in obtaining and utilizing clinic‑based and community‑based services that support individual recovery needs.
- Assist patients by identifying community resources and encouraging participation to engage in healthy social activities, employment opportunities, or provide other assistance as needed.
- Observe behaviors and evidence of general well‑being and discuss observations with the patient.
- Assist patients in developing empowerment skills through self‑advocacy.
- Ensure timely and appropriate documentation as required by company, state, and federal policy.
