• Facilitate completion of care evaluations in nephrology offices, dialysis clinics, or via telehealth
• Perform outbound calls for scheduling, reminders, and rescheduling of care evaluations
• Welcome new patients and collect information on their concerns to escalate as needed
• Maintain accurate documentation and communicate updates to the care team
• Coordinate care by ensuring access to eligible resources and assisting with referrals

• High School Diploma or GED required;
  Associate’s or Bachelor’s Degree preferred
• Minimum of 2 years of professional experience in customer service, patient-facing, or telephonic outreach roles
• Prior healthcare experience preferred
• Intermediate proficiency in Microsoft Word, Excel, PowerPoint, and Outlook
• Exceptional verbal and written communication skills, along with strong organizational abilities

• Experience in a healthcare setting or with telehealth services
• Familiarity with motivational engagement techniques
• Strong time management skills and ability to work autonomously
• Experience in coordinating care or working with chronic conditions
