Make a meaningful impact every day as a Center Well Home Health nurse. You’ll provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working closely with a dedicated team of physicians and clinicians, you’ll develop and manage care plans that support recovery and help patients get back to the life they love.

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
• Coordinate with physicians to develop initial treatment plans, revise based on assessments, and teach patients per physician orders under Clinical Manager supervision.
• Collaborate with a multidisciplinary team (Physical Therapists, Occupational Therapists, Home Health Aides, external providers) to deliver coordinated care.
• Report patient care, condition, and progress to physicians and Clinical Manager continuously.
• Implement care plans with patients and families to achieve optimal resolution of needs/problems.
• Oversee and supervise Home Health Aides, Certified Aides, and Personal Care Workers, providing written care instructions and monitoring documentation completion.
• Discharge patients with physician and Clinical Manager consultation, preparing necessary clinical documentation.
• Prepare medical documentation for all patients, including case conferences, medication orders, re-certifications, progress updates, and care plan changes.

• Diploma, Associate, or Bachelor’s Degree in Nursing.
• Minimum of one year of nursing experience preferred.
• Strong background in med‑surg, ICU, ER, and acute care.
• Experience in home health is a plus.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Strong organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation.

Pay Range
• $45.00 - $63.00 per visit/unit
• $70,500 - $96,900 per year base pay
• $77,200 - $106,200 per year upfront estimate

Scheduled Weekly Hours – 40 hours per week
