Position Overview:
Successful candidate will have Home Health experience.
This position, in cooperation with Primary Care Physician, hospitalist and hospital staff, facilitates the Home Care and/or Hospice admission process. This position is responsible for attending hospital rounds, facilitating orders, assessing patient needs, including level of care, Durable Medical Equipment (DME), comfort medications and transportation. This position enters referral into electronic medical record and completes referral process and admits patient as appropriate.
This position forwards hospital documents to hospice and communicates with hospice and hospital staff. This position provides education to hospital staff and physicians. This position is able to make home visits to provide patient care coverage as the business need requires.
Job Description:
EDUCATION:
* Graduate from an accredited licensed vocational nursing school
CERTIFICATION & LICENSURE:
* LVN-Licensed Vocational Nurse
* BLS-Basic Life Support Healthcare
* DL-Valid Drivers License Class C or B
* AUTO-Automobile Insurance
TYPICAL
EXPERIENCE:
* 5 years recent relevant experience
SKILLS AND KNOWLEDGE:
* Must have clinical knowledge of disease entities common to hospice/home health.
* Commitment to teamwork and customer service, both internally and externally
* Collaborative and problem-solving skills with customers and management. Organizational skills.
* Must have telephone, interpersonal and written communication skills.

Pay Range is $42.07 to $54.69 / hour
The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate's experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health's comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
