Overview
Physician Assistants / Nurse Practitioners – Saratoga Hospital Medical Group – Endocrinology & Diabetes. Sign‑On Bonus available.
Compensation: $118,000 – $145,000 per year. Tiered structure based on experience.
Our practice includes five endocrinologists, four APPs, and a team of diabetes educators. We are building our APP team and would like to grow to six or seven members.
The provider will manage care for diabetes patients and others with defined endocrine diagnoses. If you are an Endocrine PA/NP, or a provider in Primary Care or another specialty with experience treating diabetic patients, please highlight your experience.
Office hours 7:30 AM - 5:00 PM, Monday–Friday. Call sharing is equal among APPs, weekdays, Monday‑Thursday only. Providers work 80 hours per bi‑weekly pay period.
Location:
Convenient, near Exit 15 on Route 9 in Wilton, with amenities and eateries nearby.
- Obtains, documents, and updates medical history. Identifies health risks and needs. Performs physical examinations and conducts preventive screening procedures based on age and history. Formulates a differential diagnosis and plan of care based on history, physical examination, and clinical findings. Identifies needs of the individual, family, or community.
- Orders and reviews diagnostic tests. Identifies, prescribes, and manages pharmacological therapies.
- Determines effectiveness of the plan of care through documentation of patient care outcomes.
- Reassesses and modifies plan of care as necessary to achieve medical and health goals.
- Makes referrals to other health professionals and community agencies. Admits/discharges to appropriate service.
- Participates in team meetings and conferences to enhance an interdisciplinary approach to health care delivery.
- Serves on departmental and multidisciplinary committees, influencing or determining policies affecting advanced practice providers.
- Participates in and develops projects and quality improvement programs, as well as standards of care and practice.
- Maintains current level of knowledge relative to professional practice.
- Maintains all required professional documentation, such as license, certifications, infection control, ACLS/BLS, DEA, and system requirements.
- Acts as resource to staff and implements educational programs, as needed.
- Experience in Endocrinology with diabetic patients. Highlight your diabetes management experience in your CV or application.
- License to practice as a Nurse Practitioner or Physician Assistant in New York State.
