Position: Nurse Practitioner/Physician Assistant Geriatric Opportunity

Nurse Practitioner / Physician Assistant:
Geriatric Medicine (Saratoga Springs, NY)

This is a full-time opportunity, 8:00 am–4:30 pm Monday–Friday.

Compensation

$116 000–$150 000 (tiered by years of experience and other factors).

Position Summary

Saratoga Hospital Medical Group – Geriatric Medicine seeks a dedicated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our geriatric medicine team. The APP will provide consultative care for older adults across diverse care settings, primarily in an outpatient consultative role but also including inpatient consults, assisted‑living visits and home visits. Working closely with an established geriatrician, the APP will deliver patient-centered care that prioritizes quality of life, value-based decision-making and collaborative goal-setting with patients and caregivers.

Key Responsibilities
  • Provide comprehensive geriatric assessments, including history, physical examination, and review of functional, cognitive, and psychosocial health.
  • Conduct falls assessments, identify polypharmacy and, when appropriate, develop deprescription plans; carry out cognitive assessments and dementia care.
  • Perform longer visits that allow for thoughtful, patient- and family-centered care.
  • Manage acute and chronic medical conditions in older adults with an emphasis on appropriate workup and diagnoses aligned with patient goals and values.
  • Elicit and document goals of care, advance care planning and shared decision‑making conversations with patients and caregivers.
  • Perform consultative evaluations in the hospital, outpatient clinic, assisted‑living facilities and patient homes as needed.
  • Coordinate care across health‑care settings, collaborating with primary‑care providers, specialists, caregivers and community resources.
  • Participate in interdisciplinary team meetings to optimize care plans for medically and socially complex patients.
  • Communicate effectively and compassionately with patients, families and health‑care teams.
  • Ensure documentation and care planning are consistent with regulatory standards and value‑based care principles.
  • Collaborate with the supervising geriatrician for case discussions, clinical oversight, and professional growth.
Qualifications
  • Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) with prescriptive authority; licensure in New York State prior to credentialing.
  • Certification and clinical experience in geriatrics, internal medicine, family practice, or related specialty strongly preferred.
  • Strong interpersonal and communication skills with patients, families and interdisciplinary teams.
  • Demonstrated expertise in geriatric syndromes, polypharmacy, functional assessment and cognitive evaluation.
  • Comfort with home visits, assisted‑living consultations and transitions of care.
  • Commitment to patient-centered, value-based care models.

Pay Grade: APP $116 000–$150 000; compensation may vary based upon overall experience, shift and location.
