Register Nurse Home Health - PRN

Description

Coordinate and provide comprehensive patient care, including health assessments, care planning, and skilled nursing services. Monitor patient conditions, promote preventive care, and ensure continuity of care by coordinating with physicians and healthcare teams.

• Conduct patient assessments, including OASIS documentation.
• Develop, implement, and evaluate individualized care plans.
• Provide skilled nursing care, including treatments, medication administration, and patient education.
• Collaborate with physicians and healthcare professionals to adjust care plans.
• Supervise and evaluate Home Health Aides and LPNs.
• Document and submit clinical notes and reports as required.
• Participate in case conferences, staff meetings, and ongoing education.
• Ensure compliance with federal, state, and Medicare/Medicaid regulations.

• RN license in Florida and graduate of an accredited nursing program.
• At least 1 year of home health experience (required).
• Knowledge of Medicare/Medicaid guidelines and home healthcare principles.
• Strong organizational and time‑management skills.
• Valid driver’s license and ability to travel to patients’ homes.
• Level II background check and current in‑person CPR certification required.
