We are hiring for a Physical Therapist to join our team in Senior Living.

Responsibilities

• Evaluate the patient to determine rehab potential, identify needs, establish an individualized treatment plan, and set goals for the team plan of care.
• Set initial short-term and long-term goals.
• Utilize equipment properly and safely, maintain inspection, maintenance, and cleaning logs, and report broken or faulty equipment.
• Maintain equipment logs for outpatient clinic equipment.
• Follow the medical regimen developed for the patient and adhere to precautions (e.g., THR, TKR, cardiac, wound care).
• Utilize safe treatment techniques such as transfers, positioning, feeding, splinting, paraffin bath, cryotherapy, ultrasound, electrical stimulation, etc.
• Complete discharge summaries on each patient at their final outpatient PT visit.

Education and Experience

• Current Physical Therapist licensure in the state of practice.
• Current CPR certification required.
• Valid driver's license, vehicle insurance, access to a dependable vehicle, or reliable public transportation.
