• Perform evaluations and develop individualized treatment plans
• Reassess effectiveness of treatment plans and attend staffing meetings and family conferences
• Perform discharge evaluations and provide referrals and education resources
• Communicate patient needs and progress to the treatment team, physicians, and families
• Supervise Physical Therapy Assistants as needed
• Collaborate with other disciplines to ensure coordination of care and enhance patient outcomes

• Minimum of a bachelor’s degree in Physical Therapy from an accredited program
• Current, unrestricted license as a Physical Therapist in the state of practice
• Current Live CPR Certification and Two-step TB skin test required
• Demonstrated knowledge of evidence-based therapy techniques
• Familiarity with assisted living, independent living, geriatrics, and/or home health settings
• Communication skills in verbal and written interactions
• Ability to lift 50 pounds and transfer patients up to 100 pounds following safety procedures
• Physical screening required prior to assignment and periodically thereafter
