Job SummaryThe Homeless Case Manager Nurse is responsible for direct and indirect patient care and education. S/he participates in the case management of home less complex clinical patients. Evaluates and improves quality assurance functions for individual patients as well as through Homeless population health measures. Part of clinical nurse team as required.
Specific Tasks / Duties Include- Assists with meeting the needs of Homeless complex medical patients, including assessment, education, referrals, and follow-up care. This may involve face-to-face, phone meetings, and review of electronic messaging and media.
- Provides nursing care including but not limited to administering medications and treatments in compliance with a medical provider’s orders, Santa Rosa Community Health’s policies, and State of California guidelines. This is inclusive of assigned functions within electronic health records.
- Assists respite staff in reviewing client’s appropriateness for respite care.
- Interprets information obtained from patient and others, including the health care team, and documents all clinical care in the patient’s electronic medical record.
- Provides general care coordination including but not limited to: patient triage, phone call‑backs, prescription refills, lab ordering and interpretation, and medical case management.
- Performs education and counseling about disease management process
- Provides consultation to Care Transition Coordinators, including occasional visits with patients outside the clinical setting.
- Maintains skills proficiency.
- Performs related duties as requested.
- Participates in ongoing training.
Minimum Qualifications
Education- Current California RN license.
- Current CPR certification.
- Two years’ experience in psychiatric nursing or direct patient care and coordination of the homeless population.
- Demonstrated proficiency with all tasks on the most current RN Homeless Case Manager competency list.
- Good knowledge of the nursing process and critical thinking abilities.
- Knowledge of electronic medical records and tracking systems.
- Fluency in English and Spanish (speak, read and write) preferred.
- Demonstrates knowledge of compliance issues within the community clinic environment.
- Demonstrates a willingness to report any incident that is unusual or incompatible with accepted clinic procedures.
- Maintains confidentiality of patients at all times by complying with HIPAA policies.
- Strong interpersonal skills; ability to be sensitive with persons of various social, cultural, economic and educational backgrounds.
- Experience with Microsoft Office applications including Outlook, Word, Excel and PowerPoint.
- Strong organizational skills with ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines.
- Strong written and verbal communication skills.
- Ability to work independently and as part of a team.
- Good judgment, problem solving and decision‑making skills.

- Travel to other office and community locations.
