A community health organization in Santa Rosa is seeking an RN Homeless Case Manager to provide patient care and support for complex medical issues in homeless populations. Main responsibilities include nursing care, care coordination, and patient education. The ideal candidate must hold a valid California RN license, possess CPR certification, and have experience in psychiatric nursing or direct care for the homeless.

Bilingual candidates (English/Spanish) are preferred. This position offers competitive hourly wages based on experience.
