NURSE - Licensed Vocational Nurse, Licensed Psychiatric Technician (LVN, LPT).

Job Title(s): Withdrawal Management Nurse, Dispensing Nurse, Licensed Vocational Nurse, Licensed Psychiatric Technician.

Department: Admissions, Outpatient, Adolescent & Teen, Perinatal, MAT Services, Residential, Detox.

Location: 2403 Professional Drive, Santa Rosa, CA & 790 Sonoma Avenue, Santa Rosa, CA.

Status: Full time, Part time, On Call, AM and PM.

Reports to: Nurse Practitioner, Nurse Director.

Classification: Non-exempt.

Compensation: $34.00 - $39.00 per hour.
Additional $2.00 per hour differential for bilingual English/Spanish speaking counselors with completion of bilingual skills assessment.

Job Summary

Center Point DAAC is seeking enthusiastic and experienced nurses to join our team. The Nurse will be responsible for the preparation and administration of medication and maintenance of records for patients during certain clinic hours as part of a team of treatment providers. Ensures compliance with Title IX regulations. The Licensed Nurse (LPN/LVN) will provide medical services to chemically dependent individuals, promoting recovery and well-being, in a supportive and therapeutic environment for clients undergoing treatment in our residential facility.

Responsibilities
  • Adherence to HIPAA, maintaining confidentiality of patients' medical and personal information.
  • Respond to emergencies and initiate action, as required.
  • Prepare patients for exams and procedures, including vital sign collection.
  • Observe, document, and record-keeping of the client’s health status.
  • Familiarity with common detox assessments (CIWA and COWS preferred).
  • Assist clients accurately with self-administering medications as prescribed by physicians.
  • Specimen collection and labeling according to policy.
  • Assist with intake and orientation processes for new patients.
  • Provide safe, effective, and efficient implementation of direct care in accordance with established policies, procedures, and standards of care.
  • Monitor patient medication use, vital signs, and other physical signs and symptoms to support the client through the withdrawal phase of treatment.
  • Liaison with on-call provider, managers, nurse director, and/or medical director as necessary.
  • Triages and provides complex care coordination for all clients.
  • Oversee duties performed by unlicensed assistive personnel.
Communication and Teamwork
  • Communicate effectively with physicians, nurses, and other healthcare professionals.
  • Collaborate with interdisciplinary team members to ensure comprehensive care.
  • Receive and present the change of shift report as assigned.
  • Notify supervisor of any changes in behavior and/or condition.
Administrative Tasks
  • Enter data into electronic health systems while maintaining data integrity and accuracy.
  • Maintain accurate and up-to-date records.
  • Assist with maintaining medical supplies and equipment.
Time and Accountability
  • Maintain good attendance and punctuality.
  • Follow through on activities and responsibilities.
  • Respond promptly to phone calls and requests for information.
  • Willingness to work varying shifts.
  • Ability to adapt to changing priorities and schedules.
  • Availability to work occasional weekends or holidays (if required).
Other Duties
  • Participate in staff meetings and training sessions as required.
  • Perform phlebotomy tasks within the scope of practice.
  • Perform other tasks as assigned by the organization.
Skills &

Position Requirements
  • Current LVN/LPT licensure in the State of California with a degree from an accredited college or university.
  • Knowledge of nursing and medical practices and procedures, and applicable laws and guidelines for medication-assisted detox protocols.
  • CPR/First Aid Certification.
  • One year of experience in a substance abuse treatment setting (preferred).
  • Able to read, write, and speak English fluently (Spanish is a plus).
  • Valid and current CA Driver License (Must meet all DMV and agency insurance clearances).
  • Familiarity with Microsoft Office (Word, Excel, Outlook) and EHR systems like Kipu, Methasoft, and Smart Care.
  • Current Tuberculosis (TB) clearance within the last year.
  • Strong people skills and the ability to establish rapport with clients and staff alike.
  • Ability to work independently as well as collaboratively within a multidisciplinary team.
  • Excellent time management skills and ability to multitask and prioritize work.
  • Strong organizational and planning skills to complete tasks in a high-quality, accurate, and timely manner.
