Summerfield Healthcare in Santa Rosa, CA is a post‑acute care facility offering long‑term care, short‑term care and geriatric outpatient services. We are seeking a Full‑Time Occupational Therapist for our in‑house rehab program.

Qualifications
  • Licensed Occupational Therapist
  • Open to applicants of all experience levels
Key Responsibilities
  • Effectively screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following regulatory and clinical practice standards.
  • Utilize standardized assessments and evidence‑based practice to support clinical interventions.
  • Provide rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administer various procedures as part of the rehabilitation plan including but not limited to ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
  • Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluate treatment outcomes for effectiveness. Modify services in relation to patient evaluation and refer as appropriate.
  • Provide consultation and counseling to patients, families, caregivers and other service providers related to the physical disorders.
  • Communicate effectively with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provide comprehensive discharge summary of services per regulatory and clinical requirements.
