Job Summary
The Staff LVN is a practitioner who demonstrates in-depth knowledge of nursing patient care and the ability to practice independently within the confines of educational preparation, physician orders, and as assigned and supervised by a registered nurse.
Job Details- Demonstrates basic technical nursing skills within scope of practice (as documented in agency skills checklist).
- Utilizes the nursing process to provide safe, therapeutic care for patients: assessment, planning in collaboration with RN, implementation, evaluation, and documentation that reflects a proficient level of clinical practice. Implements established plans of care, follows frequency, works toward established goals, and reports any necessary changes.
- Collaborates, cooperates, and coordinates with RN case manager/supervisor to assure effective, consistent management of clients. Works as a collaborative member of the treatment team and reports any changes in client status to the case manager/supervisor in a timely manner.
- Documents services rendered and client response. Documentation is complete, accurate, timely, and legible. Assures client charges for supplies are accurately assessed and submitted.
- Attends required meetings and in‑services, responsible for obtaining and demonstrating knowledge of information covered.
- Demonstrates responsibility for personal and professional growth.
- Complies with safety, infection control, CHW Integrity Program, and other organizational policies. Complies with the organization’s dress code. Arrives at work on time as defined by the Attendance and Timeliness Policy and attends work consistently.
- Actively supports the organization’s mission, philosophy, and strategic plan. Demonstrates good time management and priority setting.
- Graduate of an accredited licensed vocational nursing program.
- Proof of car insurance for the automobile being used.
- California Driver License (CA DL).
- Current and valid LVN (CT LVN).
- State licensed LVN with at least one year of nursing experience.
- Med/Surg or acute hospital experience.
- Home health experience.
- Ability to work within a case‑managed model of patient‑care delivery.
- Proficient intermediate nursing skills.
- Caring, compassionate attitude toward others.
- Willingness to actively participate in agency task forces and committees.
- Ability to organize and prioritize work efficiently.
- Clear, effective written and oral English skills with active listening skills.
- Calm and efficient reaction in emergencies.
- Excellent interpersonal skills and ability to work with others across all agency departments.
