Social Worker/Service Coordinator II Children #2118 – Tri‑Counties Regional Center
Within a team environment, provides person‑centered consultation, advocacy, resource information and service coordination for people with developmental disabilities who require more involved and specialized assistance.
SpecificJob Duties
- Support individuals we serve by maintaining regular contact and conducting Individual Family Service Plans (IFSP) and Individual Program Plans (IPP).
- Participate in Interdisciplinary Team (IDT) staffings and attend community meetings as needed, including Individual Education Plan (IEP) and day program meetings.
- Provide information and referral to generic services and resources in the community, including schools, recreational options and local support groups, supportive employment/ habilitation funded day programs, local agencies (C.C.S., Department of Rehabilitation, Social Security, Medi‑Cal, IHSS), and specialist physicians/specialists (speech therapist, occupational therapist, physical therapist).
- Coordinate and secure Tri‑Counties Regional Center funded services, including behavior services, training for independent living, supportive living; out‑of‑home placement, in‑home and out‑of‑home respite; infant and adult day programs; durable medical equipment and specialized services, non‑Medi‑Cal funded services.
- Conduct strength‑based assessments that communicate an understanding of the unique issues facing family members.
- Involve individuals we serve and family in all aspects of person‑centered planning and support activities.
- Design, document, implement and monitor services and supports in a timely manner.
- Facilitate planning team progress, identifying outcomes, needed services and supports.
- Develop and maintain a network of vendors and community agencies.
- Provide relevant information and training about developmental disabilities, services and resources.
- Use effective communication and interpersonal skills that consistently respect children and adults with developmental disabilities and their families.
- Demonstrate operative knowledge of family/natural support and private/public resources and integrate these into service planning resulting in community‑based outcomes.
- Demonstrate understanding of developmental disabilities and how families are impacted.
- Demonstrate knowledge of a variety of approaches to intervention and support.
- Demonstrate knowledge of entitlement and benefit programs.
- Demonstrate knowledge of the Tri‑Counties Regional Center system, the legal system, individual civil rights and overall advocacy.
- Work cooperatively and collaboratively as a team member.
- Develop and maintain good relationships with community representatives.
- Participate in professional growth and development through attendance at in‑service training activities, conferences and other job‑related agency approved events.
- Conduct all activities in a professional and ethical manner.
- Additional or different functions may be assigned from time to time.
- Knowledge of developmental disabilities.
- Awareness of entitlement and benefit programs and public benefit programs.
- Problem‑solving ability.
- Ability to make verbal and written presentations to groups.
- Organizational and time‑management skills.
- Ability to work independently and within a team.
- Responsive to supervision.
- Able to understand and adhere to applicable laws, regulations, policies and procedures.
- Master’s or Bachelor’s degree in human services field or a related area, with 2 years of applicable experience in mental health or social work.
- 2 years experience in case management within a related field.
- 1 year experience with individuals with developmental disabilities preferred.
- Valid California driver’s license and reliable, insured vehicle or equivalent means of transportation.
- Able to travel to any location at any time.
