Overview

Central Coast Home Health and Hospice is a fast-growing and locally owned organization with an excellent reputation throughout San Luis Obispo and Santa Barbara counties. We are seeking a full-time Certified Home Health Aide (HHA) to join our team in San Luis Obispo
- Northern Santa Barbara County (Santa Maria - Lompoc).

The HHA is part of the interdisciplinary team and performs services for patients as necessary to meet the patient’s personal needs and promote comfort. The HHA is responsible for observing the patient, reporting observations, and documenting care to promote quality, continuity, and safety of the patient’s care.

Work schedule: Approx 8am to 5pm; must have availability on weekends.

• Observe patients and report observations to the care team
• Document observations and care performed to promote quality, continuity, and safety of care
• Assist patients with personal needs in accordance with the interdisciplinary care plan

• Valid Home Health Aide Certification (Required)
• Reliable vehicle to drive for work
• Current Driver’s License and clean driving record
• Auto insurance coverage
• CPR certified (Healthcare Provider)
• Home Health and/or Hospice experience preferred
• Understanding of hospice philosophy and issues of death and dying
