Summary

The primary responsibility of the Patient Navigator is to support the licensed providers with the management of their patient panel. The Navigator establishes relationships with patients, introduces them to the patient‑centered care team, and ensures timely treatment and clear communication about the plan of care and follow‑up services.

Essential Duties and Responsibilities
  • Scrubs and prepares patients’ EMR up to 3 days prior to visits.
  • Follows standard work outlined in the pre‑visit planning process.
  • Reviews providers’ orders/requests and manages order completion for referrals, labs, diagnostics, etc.
  • Records findings from order management onto the appropriate section in the EHR.
  • Reviews immunization status and preventive healthcare guidelines for each patient.
  • Calls patients to review and update information for upcoming consultations.
  • Communicates with patients to identify barriers to appointments and follow‑up care.
  • Follows provider recommendations for specialty referrals.
  • Assists in reviewing, entering, and discharging abnormal lab results under RN Care Coordinator direction.
  • Assists with follow‑up on abnormal results reviewed by the provider, ensuring notifications and additional appointments or tests as needed.
  • Provides abnormal results information to external sources such as Public Health.
  • Communicates operational problems to Clinical Services Supervisor, Associate Health Center Manager, Health Center Manager, and Regional Operations Manager.
  • Participates actively in staff meetings and daily huddles.
  • Follows up on all patient and provider test action messages in the EHR task box regarding abnormal results.
  • Promotes patient safety and adheres to standards from HEDIS, TJC, NCQA, PCMH, and UDS.
  • Demonstrates appropriate care knowledge for all patient age groups and adherence to domestic violence, child and dependent abuse protocols.
  • Observes all safety policies, infection control rules, and confidentiality requirements.
  • Works effectively with diverse populations and maintains confidentiality of patient information.
Supervisory Responsibilities

This position has no supervisory responsibilities.

Qualifications

Education and Experience

Graduate of an accredited Medical Assisting school with at least 1 year of related experience.

Language Skills

Ability to read, write, and speak English; bilingual English and Spanish preferred.

Computer Skills

Proficient in Microsoft Office (Word, Excel, PowerPoint); experienced with word processing, spreadsheets, and email.

Certificates, Licenses, Registrations

Valid California Driver’s License (Class

C). Current CPR (BLS) certification; if not certified at hire, must obtain within 30 days.

Other Requirements

Pass criminal history background check and drug screen upon hire. Annual health exam, TB test clearance or chest x‑ray, proof of immunity to MMR, Varicella, Hepatitis B, Tdap vaccine, and influenza vaccination or signed declination. Flu mask mandatory during flu season.

Physical Demands

Regular use of hands, reach, conversation; frequent sitting; occasional standing and walking; lift up to 10 lbs regularly, up to 25 lbs occasionally. Vision: close, distance, color, peripheral, depth perception. Adjustable focus.

Work Environment

Exposure to moving mechanical parts, risk of electrical shock; moderate noise level.

Schedule

Flexible work schedule may include evenings, weekends, and travel as needed.
