POSITION SUMMARY: The Clinician II will collaborate with physicians, staff, and other healthcare professionals to provide seamless patient care within the clinic setting. The nurse is a vital part of the care team, ensuring safety, adherence to best practices, and high-quality standards across the continuum of care. The Clinician II coordinates various patient wellness and chronic illness activities for the entire patient population of the clinic.
Success is measured by outcomes such as Patient Satisfaction, ACO, HCC, and clinic quality dashboards. The Clinician II also serves in a clinical leadership role, utilizing critical thinking and creative problem-solving to ensure positive clinical outcomes.
REQUIREMENTS
MINIMUM QUALIFICATIONS:
- EDUCATION: Graduate of an accredited Registered Nursing program. RN required; BSN preferred.
- CERTIFICATIONS/LICENSES: Licensed as a Registered Nurse by the State of New Mexico. BLS Certification through the American Heart Association required or within 2 weeks of hire.
SKILLS:
- Knowledge and practical use of good business English, spelling, arithmetic, and practices.
- Effective written and verbal communication skills.
- Demonstrates effective teaching techniques, applying adult learning principles.
- Ability to coordinate and implement educational materials for patients and support systems.
EXPERIENCE: One year of clinical nursing experience preferred.
NATURE OF SUPERVISION:
- Responsible to: CSVCG Dyad.
- In conjunction with clinic leadership and dyads:
- Supervises daily activities of medical assistants.
- Ensures timely orientation and competency completion for clinical support staff.
- Ensures uninterrupted clinical care.
- Provides feedback for annual performance evaluations.
ENVIRONMENT:

PHYSICAL REQUIREMENTS: Full range of motion, including handling and lifting patients, manual dexterity, and eye-hand coordination. Standing and walking for extended periods. Occasionally lifts up to 50 pounds. Works under stressful conditions or irregular hours. Exposure to bodily fluids, communicable diseases, toxic substances, radiation, and medications common in clinics.
