Mental Health Therapist

The Mental Health Therapist provides assessment, treatment, and collaboration with clients, families, and community agencies as part of a medically integrated care team.

• Deliver a minimum number of client visits weekly and conduct mental health assessments.
• Support clients in overcoming barriers to engagement by coordinating with care team members across various systems.
• Develop and implement individualized care plans, maintaining clear documentation.
• Participate in supervision, team meetings, and case conferences, and collaborate with colleagues and other providers.
• Provide evidence-based clinical treatment and engage in ongoing training and consultation.
