All positions require on‑site visits to patients’ homes; remote work is not available unless specifically noted.

• Perform home evaluations, develop care plans, and coordinate care with PTAs and physicians.
• Provide therapeutic interventions to restore function and prevent disability.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure quality of care.
• Evaluate and report patients’ progress to MDs.

• Flexible, self‑scheduled work hours.

• Company‑issued Ipad or laptop, phone number, and annual Med Bridge membership for CEU training.
• Vehicle reimbursement of $0.70 per mile.

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver’s License.
• Knowledge of local community resources.
• Understanding of individual development and cultural influences on health care.
• Knowledge of state and federal home‑care regulatory guidelines preferred.
• Experience in home health care highly preferred; high acuity patients a plus.
• Experience working with interdisciplinary teams and case management fundamentals highly desired.
• Strong self‑directed, empathetic, critical thinking, interpersonal, leadership, and organizational skills.
