This role is part of our Enhanced Care Management (ECM) team — a bold new service line designed for our patients who face multiple barriers to wellness. You’ll work side-by-side with our behavioral health clinicians and administrative team to deliver personalized, equity-driven care.

You will report to the Director of Behavioral Health.

What You’ll Do

Across all interventions, your focus is on:

  • Building trusted relationships
  • Partnering with people and providers in creating clear, actionable care plans
  • Centering equity, autonomy, and integrity in all interactions
  • Bringing flexible care to people where they are – in the community, where people reside, attending appointments and more

This is a direct outreach, fieldwork position. Approximately 80% of your time will be traveling independently throughout Santa Cruz and San Benito counties, with the remaining 20% of your time working in our Santa Cruz office for meetings and administrative work.

1. Care Coordination

  • Navigate complex health and social systems including advocacy for people accessing the right care from the right person at the right time
  • Ensure seamless of care across medical, behavioral health (including struggles with substance use disorder), and social services by bridging complex system webs
  • Support people in maintaining access to Medi-Cal and other critical benefits

2. Support for Change

  • Use motivational interviewing and trauma-aware approaches to support people who are stuck, ambivalent, or struggling to follow through on care plan goals
  • Collaborate with the therapy team and other providers to address barriers and build momentum

3. Crisis Care Coordination

  • Coordinate transitions into and out of high levels of care (psychiatric hospitalization, residential SUD treatment, etc.)
  • Provide family and systems support during crisis or reentry

  • Bring experience working with people with complex needs, especially those navigating multiple systems
  • Have a strengths-based, empathic approach to care
  • Have experience using self-awareness in your provision of care to assess your own needs and boundaries
  • Are flexible and excited to be part of standing up a new service in an integrated model

Preferred Qualifications

  • Lived experience and relevant professional experience and/or Bachelor’s degree in social work, public health, psychology, or related field
  • At least 2 years of experience in care management, care coordination, or a related role
  • A driver’s license and a reliable, consistent method of personal transportation covered by your own personal insurance policy
  • Familiarity with Medi-Cal and community-based health systems preferred
  • Project management experience, involvement in launching new programs, and tech savvy a plus!
