Home Health Physical Therapist
- Conduct in‑home PT evaluations and follow‑up visits.
- Design and implement individualized rehabilitation and treatment plans.
- Improve patients’ strength, balance, mobility, and functional independence.
- Educate patients and caregivers on safe movement and equipment use.
- Document visits using Well Sky Kinnser EMR.
- Coordinate care with physicians, RNs, case managers, and the interdisciplinary team.
- California‑licensed Physical Therapist (PT)
- CPR certification (AHA‑approved)
- At least 1 year of clinical experience (Home Health experience preferred)
- Organized, compassionate, and reliable
- Valid driver’s license, auto insurance, and reliable transportation
- Strong communication skills and clinical decision‑making
- Flexible independence to set your schedule
