• Conduct in‑home PT evaluations and follow‑up visits.
• Design and implement individualized rehabilitation and treatment plans.
• Improve patients’ strength, balance, mobility, and functional independence.
• Educate patients and caregivers on safe movement and equipment use.
• Document visits using Well Sky Kinnser EMR.
• Coordinate care with physicians, RNs, case managers, and the interdisciplinary team.

• California‑licensed Physical Therapist (PT)
• CPR certification (AHA‑approved)
• At least 1 year of clinical experience (Home Health experience preferred)
• Organized, compassionate, and reliable
• Valid driver’s license, auto insurance, and reliable transportation
• Strong communication skills and clinical decision‑making
