Essential Duties and Responsibilities
  • Client Engagement:
    Work as part of a multi-disciplinary team to assess client needs. Design, modify and implement individualized treatment plans.
  • Design and modify individualized treatment plans with the support of the program manager.
  • Document changes in client's conditions and take necessary action.
  • Facilitate trainings, health education and medication management.
  • Participate in Circle of Support, IPP, ISP and other meetings.
  • Coordinate with medical providers and pharmacy for ongoing care. Assist with making and transporting clients to medical and other appointments as needed.
  • Educate and support clients with behaviors to adhere to their overall care plan.
  • Serve as a liaison between TASC, client, the IPP team, and generic resources following best practices and point of contact standards. Assist client with maintaining open communication.
  • Identify and utilize community resources to meet client needs.
  • Prepare reports.
  • Attend planning meetings.
  • Training and collaboration with staff and other service providers.
  • Train, supervise, and evaluate behavior therapists.
  • Work well under pressure, ability to prioritize, and has strong time management skills.
  • Foster team engagement and growth.
Program Management
  • Schedule client coverage and identify emergency coverage as needed.
  • Efficiently utilize the time record keeping system, including claiming hours for direct care staff and ensuring payroll information is accurate.
  • Perform weekly in person visits, direct observation and support of clients and teams.
  • Prepare and submit SIR’s and appropriate documentation to funding agencies, APS, and licensing within the specified time period with support of a program manager.
  • Document provided services for monthly billing.
  • Review and analyze daily notes and data.
  • Assist in crisis management as needed.
  • Coordinate and maintain both client based and administrative responsibilities.
  • Draft and present disciplinary actions to employees per TASC procedures.
  • Provide support as needed to other departments per Director instruction.
TASC Leadership
  • Represent TASC within the community.
  • Participate on community boards and/or committees in order to enhance community knowledge of TASC as well as provide TASC with relationships with additional community resources.
  • Foster team engagement and growth.
  • Other duties as assigned.
Qualifications
  • Active RN/LVN/LPT licensure in good standing.
  • At least 3 years of experience providing treatment to adults with developmental disabilities.
  • Strong computer skills, including demonstrated success with Microsoft Office Suite; strong skills and comfort with internet applications and social networking sites.
  • Must be able to successfully complete First Aid/CPR.
  • Maintain all certifications up to date.
  • Must have current California Driver’s license/Insurance and clean DMV.
  • Must receive criminal record clearance through the DOJ.
