Role DescriptionThis is a full-time, on-site Case Manager role located in San Jose, CA. The Case Manager will provide comprehensive support to clients experiencing or at risk of homelessness. Responsibilities include conducting assessments, creating individualized service plans, connecting clients with community resources, advocating on their behalf, and providing ongoing support to help them achieve housing stability. The role involves collaboration with multidisciplinary teams to deliver tailored services that focus on meeting client needs effectively and compassionately.
Qualifications- Case management, counseling, and crisis intervention skills
- Familiarity with housing programs and community resources
- Strong interpersonal, communication, and active listening skills
- Time management, organizational, and problem-solving skills
- Proficiency in documenting case notes and maintaining accurate records
- Ability to work collaboratively in a multidisciplinary team and with diverse populations
- Bachelor s degree in Social Work, Psychology, or a related field (or equivalent experience)
- Proficiency in a second language is an asset
