RN Case Manager, Hospice or Per-Diem

We are hiring RN Case Managers to join our Hospice Team in Santa Barbara. We have Per-Diem and Full-Time opportunities available.

$81 per/routine and $141 per/eval visits.

• Have flexible hours and working close to home (reduce your commute!)
• Complete initial and on-going comprehensive assessments of the patient’s needs
• Provide service and /or treatments requiring substantial and specialized nursing skills
• Initiate appropriate preventative and rehabilitative nursing procedures
• Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs

• Graduate of an approved school of professional nursing.
• Current, active license in the state of California Board of Nursing,
• Current CPR, health clearance, TB clearance.
