Job Summary
Within a team environment, provides person-centered consultation, advocacy, resource information and service coordination for people with developmental disabilities who require more involved and specialized assistance.
People with developmental disabilities served by Service Coordinator 2’s require specialized assistance, including but not limited to:
- Students in active transition due to graduating from school in one to two years.
- Individuals in residential placement or living independently.
- Newly eligible individuals, transfers from other Regional Centers, and reactivated cases.
- Individuals with dual diagnosis, and cases requiring clinical expertise.
- Self-determination and Medicaid Waiver cases, Social Security/Medi-Cal problems that require assistance with advocacy and appeal process.
- Support individuals we serve by maintaining regular contact and developing Individual Family Service Plans (IFSP) and Individual Program Plans (IPP).
- Participate in Interdisciplinary Team (IDT) staffings and attend meetings in the community as needed, including Individual Education Plan (IEP) and day program meetings.
- Provide information and referrals to generic services and resources in the community, including schools, recreational options and local support groups.
- Supportive Employment/Habilitation funded day programs.
- Coordinate with local agencies (C.C.S., Department of Rehabilitation, Social Security, Medi-Cal, IHSS) and specialist physicians/therapists (speech therapist, O.T./P.T.).
- Coordinate and secure TCRC funded services, including behavior services, training for independent living, and supportive living; include out-of-home placement, in-home and out-of-home respite; infant and adult day programs; durable medical equipment and non-Medi-Cal funded services.
- Conduct strength-based assessments that reflect understanding of the unique issues facing family members.
- Involve individuals we serve and their families in all aspects of person-centered planning and support activities.
- Design, document, implement and monitor services and supports in a timely manner.
- Facilitate planning team progress, identify outcomes, and determine needed services and supports.
- Develop and maintain a network of vendors and community agencies.
- Provide relevant information and training about developmental disabilities, services and resources.
- Use effective communication and interpersonal skills that respect children and adults with developmental disabilities and their families.
- Demonstrate knowledge of family/natural supports, private/public resources, and integrate these into service planning to achieve community-based outcomes.
- Demonstrate knowledge of developmental disabilities and how families are affected; understand various intervention and support approaches; knowledge of entitlement and benefit programs; familiarity with the Tri-Counties Regional Center system, the legal system, civil rights, and advocacy.
- Work cooperatively as a team member and develop relationships with community representatives.
- Participate in professional growth through in-service training, conferences and agency-approved events.
- Conduct all activities in a professional and ethical manner.
- Additional or different functions may be assigned from time to time.
- Knowledge of developmental disabilities.
- Awareness of entitlement and public benefit programs.
- Problem-solving ability.
- Ability to make verbal and written presentations to groups.
- Organizational and time-management skills.
- Ability to work independently and within a team.
- Responsive to supervision.
- Able to understand and adhere to applicable laws, regulations, policies and procedures.
- Ability to read/speak/write Spanish and English (preferred).
- Master’s or Bachelor’s degree in a human services field or related area, with 2 years of applicable experience in mental health or social work.
- 2 years experience in case management within a related field.
- 1 year experience with individuals with developmental disabilities (preferred).
