RN Case Manager; PD

Collaborates with the patient and their families, the patient’s physicians, and all care providers in coordinating the care of the patient, utilizing skills of advocacy, communication, and education. Identifies, facilitates, and coordinates the appropriate services, resources, providers, and facilities throughout the continuum of care.

Requirements:

• REGISTERED NURSE WITH AT LEAST 5 YEARS OF ACUTE MEDICAL/SURGICAL NURSING EXPERIENCE AND 2 YEARS OF CASE MANAGEMENT EXPERIENCE
• MINIMUM 3 YEARS RN RECENT ACUTE CARE EXPERIENCE
• BASIC COMPUTER SKILLS
• KNOWLEDGE OF CASE MANAGEMENT COMPONENTS: UTILIZATION REVIEW AND DISCHARGE PLANNING
• CA RN LICENSE & BLS CARD from American Heart Assoc.
