The Cambodian Family (TCF) Community Center is a community-based, multi-cultural, non-profit organization, located in Santa Ana, California. Our mission is to provide refugee and immigrant families with the opportunities to develop the knowledge, skills, and desire for creating better health and well- being in their lives. We accomplish this by providing programs and services in three main areas: (1) community health and mental health;

(2) youth program; and (3) civic engagement, immigration, and advocacy.

Description

Title: Health Navigator/Case Manager (Bilingual English/Spanish)

FLSA: Non-Exempt, 40 hours/week (Full-Time). This position is a grant-funded position.

Reports to: Director of Health & Mental Health Programs

Pay Range or Rate & Benefits: $20-22/hour - plus excellent benefits package (medical, dental, vision, 403b retirement matching plan, paid time off such as vacation and sick)

Summary

Working under the supervision of the Director of Health and Mental Health Programs, a health navigator position is needed to join TCF’s response efforts regarding Medi Cal and other health insurance enrollment to our underserved community members, especially 50+ community members regardless of their immigration status. The health navigator/case manager will provide outreach, education, and application assistance to Medi Cal candidate.

The successful candidate will also provide case management services to residents served through our Resident Support Services program.

Essential Functions Health Access Assister (50%)
  • Screens prospective and current members for health insurance/public benefit programs and other social services
  • Assist clients in completing application and applying for benefits such as Medi Cal, Medi Care, Cal Fresh, Unemployment, etc. (Training will be provided)
  • Provides educational health information and assists members in accessing/utilizing medical and social services while encouraging prevention and awareness.
  • Actively outreaches to community and partners to enroll new clients into healthcare coverage.
  • Acts as a patient advocate for clients requiring assistance with healthcare/provider issues for the purposes of facilitating access to health services and insurance.
  • Using a CRM follows-up on approval, utilization, retention, medical home, and resource & referrals.
  • Organizes workload throughout the day/week/month to meet annual goals and objectives.
  • Perform other work-related duties as required by Program Director and Executive Director
  • Provide translation of documents and interpretation (English/Spanish)
  • Provide referrals/linkages to health/social service agencies/community resources
  • Plan, host, and promote community events throughout the year
  • Create outreach and marketing materials and post notices or make phone calls when necessary to residents, property management and project managers with regards to information and resources
  • Assist in hosting and coordinating meetings between residents and property management team
  • Assist residents in accessing social and support services such as housing, food, cash/rent assistance, etc.
  • Organize one-on-one or group activities to promote healthy behaviors and lifestyle changes
  • Various office and customer service tasks as assigned
  • Keep track of office visits and services that are offered; prepare and submit monthly reports
  • Communicate with Community Impact Manager and TCF Program Director weekly, and
  • Perform other work-related duties as required by Program Director and Executive Director
Minimum Qualifications - Knowledge,

Skills and Abilities

Required
  • Must have at least a Bachelor’s degree in Public Health/Health Sciences, Social Work, or any other fields
  • Must have at least one year of prior experience working with refugees and immigrants or other underrepresented communities in health education, health accessing, or health care setting
  • Must be bilingual in English/Spanish
  • Ability to interact effectively with residents, youth, and their families
  • Good communication and writing skills
  • Excellent attention to detail
  • Must have the ability to work independently and in a team
