Job

Overview

The Community Health Worker (CHW) is a trusted community member who helps address chronic conditions, preventive health needs, and health‑related social needs. CHWs understand the challenges at‑risk or vulnerable patients face and help them navigate the system. They assist patients in confronting social, medical, and community barriers that hinder optimal health. CHWs facilitate health care navigation, specialty referrals, primary care appointments, routine care, preventive screenings, cultural brokerage, financial services, and culturally appropriate health information on chronic disease management and prevention.

Minimum Requirements
  • A High School Diploma or GED.

  • Minimum of 2 years of experience in health care or community outreach settings.

  • Bilingual English/Spanish/Mandarin/Cantonese, depending on location.

  • Valid BLS certification or higher, per the American Heart Association or the American Red Cross.

Compensation

$26.91 - $33.53 hourly
