Description
Responsibilities- medical necessity screening
- facilitating multi-disciplinary patient care conferences
- making the appropriate referrals to other departments (e.g., nutrition, PT/OT/ST)
- referring complex social issues to Social Service
- communicating with patients and their families about the plan of care
- collaborating with physicians, office staff and ancillary departments (e.g., lab, pharmacy)
- participating in weekly Complex Case Review
- clear, complete and concise documentation in eCCM
- maintaining accuracy of patient demographic and insurance information
- identifying and documenting potentially avoidable days
- identifying and reporting over and under utilization
Training commensurate with nursing licensure. BSN, preferred.
Experience:
RN with minimum 5 years bedside experience or LPN with 10 years experience in the relevant clinical speciality, preferred (e.g., case manager for pediatrics patients should have pediatric nursing experience). Supervisory experience and Case Management experience, preferred. Must obtain/ maintain CPR certification according to CCH policy. Licensed as a Registered Nurse or LPN. Case Management Certification, preferred. Other Minimum Abilities / Aptitudes:
Excellent organizational skills;
Excellent verbal and written communications skills;
Ability to lead and coordinate activities of a diverse group of people;
Demonstrated critical thinking and problem-solving skills;
Computer literate;
Able to work on smoke-free campus.
SCHEDULE is20 hoursper week, evenings; includes some weekend and holiday rotation.
