Role Description

This is a full-time, PATH Outreach Case Manager role, based in York County and based primarily in Sanford and surrounding towns. The PATH Outreach Case Manager will work closely with individuals experiencing homelessness to provide support in accessing stable housing, basic needs assistance, and mental health or substance use treatment. Key responsibilities include conducting community outreach, building connections with clients, assessing individual needs, coordinating care services, and tracking progress to ensure effective outcomes.

The case manager will also collaborate with local agencies to enhance support networks for the population served.

Qualifications
  • Strong interpersonal, communication, and active listening skills
  • Experience in case management, resource linkage, and service coordination
  • Knowledge of services related to mental health, substance use treatment, and homelessness support
  • Ability to conduct effective outreach and build trust with diverse populations
  • Proficiency in documenting and tracking client progress
  • Ability to work independently and as part of a team
  • Bachelor’s degree in social work, psychology, or a related field preferred
  • Experience working with vulnerable populations is highly valued
  • Valid driver’s license and reliable transportation required
