Registered Nurse - Home Health - SLC

Position: Registered Nurse - Home Health - SLC County
Active Home Health is seeking Part-Time and PRN Home Health RNs to serve patients throughout Salt Lake County.

Job Requirements:

* Visit and assess patients to complete an initial comprehensive OASIS or assessment and ongoing assessments through end of care.

* Chart all clinical documentation in our EHR software

* Create and manage patient plan of care

* Educate and give instruction to patient, family, and caregivers as appropriate per patient care plan.

* Case manage the patient episode or benefit period, ensuring that all cares are performed and documented

* Provides medication education and management per physician order

* Works hand-in-hand with the IDT to ensure patient goals are met and appropriate outcomes are achieved

* Shares on-call duties

* Maintains strong communication with patient, family, caregivers, and the interdisciplinary teams

Job Qualifications:

* Registered nurse with current license to practice professional nursing in the state of Utah

* Polite and compassionate

* Excellent observation, problem solving, verbal and written communications

* Maintains a current CPR certification

* 1 year Home Health or Hospice experience preferred

* Must be a licensed driver with an automobile that is insured, registered and in good working order

* Good communication and smartphone skills

* Ability to pass a background check
