Advanced Practice Provider Family Medicine role at Intermountain Health. This is a part-time (30 hours per week) opportunity at Alta View Hospital, Sandy, UT.

$56.00/hr - $87.00/hr

Family Medicine APP (Family Medicine) – Part-time (30 hours a week) – Alta View, Intermountain Health – Sandy, UT

• Provide evidence-based, patient-centered treatment plans for chronic and acute medical conditions.
• Order and interpret tests, order medications, provide patient education, and coordinate appropriate post-discharge referrals.
• Document initial assessments and follow-up visits on the day of service with the appropriate billing codes.
• Collaborate with the attending psychiatric provider and interdisciplinary team (nursing, social work, pharmacists, navigators) as needed.
• Consult with hospitalists and other specialists as needed.
• Assist with coordination for transfer to a higher level of care within the hospital when appropriate to ensure patients receive the appropriate level of care.
• Support new behavioral health clinic initiatives and related patient flow as applicable.

• Two (2) or more years of experience preferred.
• Current state license as a Physician Assistant or Advanced Practice Registered Nurse (APRN).
• Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement).
• Eligible for State Controlled Substance License II through V.
• Eligible for Federal DEA Certificate II through V.
• BLS Certification for Healthcare Providers.
• Other certifications as required (e.g., ACLS, PALS, TNCC, NRP).

• Masters degree from an accredited institution. Degree will be verified.
